Registration for the 2008
The 2008 on-line Registration will
begin April 1, 2008
DEADLINE FOR REGISTRATION WILL BE JUNE 20TH IN ORDER TO ALLOW US TO MAKE TEAMS, ORDER UNIFORMS AND GATHER COACHES & REFS.

CHECKLIST FOR REGISTRATION
At registration you will receive a registration
form.
Please review the registration form for accuracy
and make additions and changes as appropriate.
The following checklist may help you to prepare
a complete registration form ready for processing:
- Ensure correct name, address and phone number.
- Provide Emergency contact information.
- Provide physician information.
- Ensure correct birth date, age (calculated
as of July 31, 2000), and, if provided, E-mail
address and Social Security Number.
- Provide name of Medical Insurance carrier,
if any.
- Ensure correct parent/guardian information.
- Provide years experience. Please note
on form if your child is smaller than his
classmates.
- Volunteer to help as a coach, assistant coach or referee!
Free training will be provided.
- Prominently note on the form any desired
information about a requested team assignment
- e.g., a coach's name or a team location.
- List siblings also playing.
- Sign and date the form. Without you signature,
emergency medical treatment could be hindered.
- If desired, make a photocopy of the registration
form for your records.
- Return the completed form, with your payment
(checks made out to AYSO), at one of the
scheduled registration sessions. Early registration
will help ensure timely team formation, (pending
coach availability).
